Please note that request forms MUST BE SIGNED and include your correct mailing address, Players’ Club account number, Social Security number and date of birth or we will not be able to provide you with a Win/Loss Statement. A copy of your Driver’s License or State Issued ID must be included to process your request. All statements will be mailed within two weeks from receipt of each completed request form.
